How we handle information, what to expect online, and what to understand before a telehealth visit.

Website Privacy Policy

Effective September 26, 2026.

This policy explains information handling on the Precision Health and Wellness public website, operated by Precision Health and Wellness LLC. It is separate from the Notice of Privacy Practices describing medical records and protected health information.

Information associated with a website visit

Loading a website sends technical information such as your IP address, requested page, browser and device information, and request time to the services delivering it. These services may maintain logs for delivery, security, reliability, and abuse prevention. Third-party features may receive the page address or referring website and use their own cookies or similar technologies.

This version of the public website has no medical intake, payment, contact-message, or patient-account form. The visit guide requests the practice's publicly listed services and prices, not patient records. Do not put diagnoses, medication lists, identification documents, payment details, or other sensitive information in public comments, social-media messages, or website URLs.

Services used by the website

  • Cloudflare: Delivers and protects the website and runs the public scheduling-information adapter.
  • OptiMantra: Provides the public scheduling information. Continuing to schedule opens OptiMantra, where you may be asked for patient, contact, payment, and intake information. This policy does not describe every aspect of OptiMantra's processing.
  • Behold and Instagram: Display the embedded Instagram feed. The embed may contact Behold, Meta, and media-delivery services when it loads. Clicking posts or profile links opens those services.
  • Soro: Supplies the article library and homepage article previews. Loading these features contacts Soro and services used to deliver article images and content.
  • Google: Supplies fonts and the embedded map on other site pages. Opening directions or Google business information uses Google's services.
  • Facebook and other outbound links: Open third-party sites when selected. Their own privacy practices apply to activity on those sites.

Some embedded features load without a separate click. This website is not a confidential channel for medical communications. Third-party tools are not made HIPAA-compliant merely by linking to them or describing them here.

Cookies, analytics, and advertising

No separate Google Analytics tag or Meta advertising pixel has been added to this website build. That does not mean the site is cookie-free or that embedded services do not collect usage information. Your browser may allow you to block cookies or third-party content; doing so may prevent feeds, articles, fonts, or maps from working. You can still telephone the practice for help.

Do not assume a browser privacy signal blocks every third-party request. For questions about particular website features or privacy settings, contact the practice before using those features.

Use, sharing, and retention

Technical information is used to deliver requested content, maintain security, troubleshoot problems, and operate website features. Providers supplying these features receive information needed to deliver them and may process information under their own terms. Information may also be disclosed when legally required. This policy does not promise a uniform retention period for logs or information held by different service providers. Contact the practice with a question about a particular record or service.

Medical information supplied through clinical systems is handled separately under the practice's clinical privacy procedures and applicable law. A request to delete website-related information does not necessarily permit deletion of a medical record that must be retained.

Your questions and choices

You may ask what information the practice holds about you and request access, correction, or deletion where applicable. The response depends on the information, identity verification, and applicable law; this policy does not promise deletion of legally required records. Contact the practice by telephone or mail using the details below. Avoid including medical information in ordinary email or social-media messages.

If you believe a child has submitted personal information through a linked service, notify the practice. This public website does not establish eligibility for pediatric care. Changes to website features or processing practices should be reflected in an updated policy with a new effective date.

These terms concern use of the public Precision Health and Wellness website. Separate clinical consent, financial agreements, and scheduling-system terms may apply to care. This website does not collect acceptance of a treatment agreement.

Education, not individual medical advice

Website articles, social posts, service descriptions, and other materials provide general information. They are not a diagnosis, prescription, individualized treatment plan, or substitute for evaluation by a qualified healthcare professional. Do not delay care because of something you read here. Viewing the website, sending an inquiry, or selecting an appointment does not by itself establish a clinician-patient relationship.

Treatment depends on an appropriate evaluation, clinical judgment, eligibility, and applicable law. A consultation does not guarantee a prescription, a particular medication, test, service, weight change, or other outcome. Educational content may discuss services that are not available or appropriate for every patient.

Appointments and availability

Visits are by appointment only; there are no walk-ins. Office hours and appointment availability may change. Available scheduling times in OptiMantra, rather than general website hours, determine what can be booked. Pennsylvania telehealth availability is subject to the patient's physical location, the provider's licensure, and clinical appropriateness.

Prices and financial policies

The visit guide reads descriptions, prices, durations, and links from the practice's public OptiMantra scheduler. Changes may take up to five minutes to appear after they are visible there. If the connection fails, use the full scheduler or call the practice.

Review the price and applicable deposit, cancellation, refund, and intake requirements displayed in OptiMantra before confirming. Ask whether labs, medication, follow-up visits, or other services are charged separately. This page does not establish a new cancellation fee, refund restriction, insurance-participation promise, or guarantee of reimbursement. Questions or discrepancies should be resolved with the practice before booking.

External services and content

Booking, embedded feeds, maps, and the article library depend on third parties and may be unavailable or change. External content is not a guarantee of accuracy, endorsement of every claim, or a promise of treatment. The practice may correct errors and update information, but website content may not always reflect the latest clinical guidance or scheduling changes.

Responsible use and content rights

Do not attempt unauthorized access, interfere with site operation, impersonate another person, or submit sensitive patient information through public channels. Practice branding and original materials belong to their respective rights holders. You may view and print information for personal use; other reuse requires permission unless otherwise allowed by law. These terms do not restrict lawful quotation, criticism, or other protected uses.

Your legal rights

These website terms do not waive patient rights, excuse professional negligence, or replace informed consent. No mandatory arbitration or class-action waiver is imposed here. Pennsylvania law applies where permitted, without displacing rights or protections that applicable law makes mandatory.

Emergencies and help

Not for emergencies. Call 911 or go to the nearest emergency department for urgent concerns. Website and social-media channels are not monitored emergency services. For nonurgent scheduling or website questions, call 724-382-7168.

Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This notice applies to Precision Health and Wellness LLC. It describes your rights, the practice's responsibilities, and permitted uses and disclosures of protected health information.

Your rights

  • Access your records: Ask to inspect or receive an electronic or paper copy of medical and billing information maintained about you. A copy or agreed summary is usually provided within 30 days, subject to lawful exceptions and extension rules. A reasonable, cost-based fee may apply where permitted.
  • Request a correction: Ask to amend information you believe is incorrect or incomplete. A request may be denied where permitted; we will explain the reason in writing, generally within 60 days, subject to lawful extensions, and explain your rights to respond.
  • Request confidential communications: Ask us to contact you in a particular way or at a particular location. We will accommodate reasonable requests.
  • Request limits: Ask us to limit use or disclosure for treatment, payment, or healthcare operations. We need not agree to every request. If you pay in full out of pocket for an item or service and ask us not to disclose it to your health plan for payment or healthcare operations, we will honor that restriction unless disclosure is required by law.
  • Receive an accounting: Ask for a list of certain disclosures during the preceding six years, excluding disclosures such as treatment, payment, healthcare operations, and other legally excepted categories. One accounting in a 12-month period is free; a permitted fee may apply to additional requests.
  • Receive this notice: Request a paper copy even if you previously agreed to electronic delivery.
  • Use a representative: A legally authorized personal representative may exercise rights on your behalf, subject to verification and applicable exceptions.
  • Complain without retaliation: Contact the practice or the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights were violated. We will not retaliate.

To exercise a right or ask about a refusal, contact Shea Whisner at the practice number or send a written request to the address in the contact section, marked “Privacy request.” We may need to verify your identity or the authority of your representative.

Routine uses and disclosures

  • Treatment: We may use and share information with professionals involved in your care, such as a laboratory, pharmacy, or referring clinician.
  • Payment: We may use and disclose information to bill and obtain payment from you or an appropriate payer. This permission does not mean the practice accepts or bills every insurance plan.
  • Healthcare operations: We may use and disclose information to administer the practice, improve care, review quality, and contact you about care.
  • Service providers: Vendors performing functions for the practice may receive information as permitted by law and, where required, under agreements that protect it.

Your choices and written authorization

You may tell us your preferences about sharing relevant information with family, friends, or others involved in your care or payment, and in disaster-relief situations. If you cannot express a preference, we may make permitted disclosures in your best interests. We may also disclose information as allowed to lessen a serious and imminent threat.

Uses or disclosures not described in this notice require your written authorization unless otherwise permitted or required by law. Most uses or disclosures of psychotherapy notes, marketing uses requiring authorization, and a sale of protected health information require written authorization. You may revoke an authorization in writing, except to the extent action has already been taken in reliance on it.

If the practice conducts fundraising, you may opt out of future fundraising communications. If Part 2-protected substance-use-disorder records would be used for fundraising, you must first receive clear, conspicuous notice and a choice about receiving those communications.

Other permitted or required disclosures

Subject to the conditions, limitations, and safeguards required by law, information may be used or disclosed for public health and safety, disease prevention, adverse-event and product-recall reporting, reporting abuse or neglect, health oversight, approved research, organ or tissue donation, coroners and funeral directors, workers' compensation, authorized law-enforcement purposes, certain government functions, and compliance with law, court orders, or legally valid requests. A subpoena does not automatically permit every disclosure.

We will provide information to HHS when required to demonstrate compliance. When a more protective law applies, it controls over a broader HIPAA permission.

Specially protected information

If we hold substance-use-disorder patient records subject to 42 CFR Part 2, we will not use or disclose those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and a subpoena, as required by applicable law. The general permissions above do not override these protections.

Pennsylvania law may impose additional protections for HIV-related information, mental-health treatment records, and substance-use-disorder information. Where applicable, we follow the stricter consent, disclosure, and redisclosure limits. For example, confidential HIV-related information is subject to designated permitted disclosures and specific written-consent requirements; a general medical-record authorization is not always sufficient.

Information disclosed to a recipient may be redisclosed and may no longer be protected by HIPAA, depending on the recipient and applicable law. Special protections, including Part 2 and Pennsylvania confidentiality requirements, may continue to apply.

Our responsibilities

We are required to protect the privacy and security of protected health information, provide notice of our legal duties and privacy practices, and follow the notice currently in effect. We will notify affected individuals following a breach of unsecured protected health information when required by law. We will not use or disclose information beyond the permissions described here without a valid legal basis or required authorization.

We may revise this notice, and permitted changes may apply to information already held as well as information received later. The current notice will be available on the website, in the office, and on request. Acknowledgment of receiving a privacy notice is separate from consent to treatment and from authorization to release records.

Telehealth Information & Consent

What telehealth means

Telehealth allows you and a healthcare professional to meet or exchange information while in different locations, using approved communication technology. The practice offers Pennsylvania telehealth when clinically appropriate. A remote visit may involve discussing symptoms, reviewing records or laboratory results, and planning care.

Benefits, limits, and alternatives

Telehealth may reduce travel and improve access, but it cannot replace every in-person examination, measurement, test, or emergency evaluation. Incomplete information, poor image or sound quality, equipment problems, or a lost connection can delay care or affect assessment. No treatment outcome or prescription is guaranteed.

You may ask questions, decline telehealth, or withdraw permission for future telehealth care. Discuss available in-person alternatives with the practice. An alternative appointment may require different scheduling, travel, or a referral. Charges for services already provided remain subject to the agreed financial policy.

Your location and safety

Be physically located in Pennsylvania unless the practice explicitly confirms it may lawfully treat you at another location. At the start of the visit, provide your identity, current physical address, and a callback number. Participate from a private, safe location; do not drive during a visit.

For an emergency, call 911 or go to the nearest emergency department rather than waiting for a telehealth response. If the connection fails, use the callback plan agreed with the clinician. If you cannot reconnect, telephone 724-382-7168; if your symptoms are urgent, seek emergency care. Agree on a callback plan with your clinician at the start of the visit.

Privacy and participation

Electronic communication carries privacy and security risks, including unauthorized access, accidental disclosure, and technical failure. Use a private connection and a device you control when possible. Tell the clinician who else is present, and ask who is participating at the clinician's location.

Visits must not be recorded without separate, explicit agreement and any legally required consent from participants. Do not assume permission to record. Medical documentation in the health record is different from an audio or video recording.

Fees and clinical follow-up

Review current visit charges and financial terms in OptiMantra before booking. Insurance coverage or reimbursement is not guaranteed. Telehealth may result in a recommendation for laboratory testing, an office visit, urgent care, or another clinician, with separate charges where applicable. Follow the plan discussed with your clinician and ask how to obtain follow-up help.

Consent acknowledgment

I have reviewed the telehealth information, understand the potential benefits, limitations, privacy risks, alternatives, and emergency instructions, and have had the opportunity to ask questions. I understand that I may decline telehealth or withdraw permission for future remote care. I understand that the clinician may recommend in-person evaluation and that a prescription or outcome is not guaranteed. I voluntarily consent to the telehealth services discussed with my clinician, subject to applicable law and the practice's approved policies.

This acknowledgment must be reviewed with the patient and documented through the practice's clinical intake process or medical record before telehealth care. Ask the practice if you have not received consent instructions through OptiMantra. Browsing this page or clicking “Book a Visit” is not consent. No acceptance box, signature field, or patient-data form is provided here.
